Care Management assists frail elderly and functionally impaired adults (age 18 and over) obtain the services necessary to allow them to remain at home safely, with a measure of independence and dignity. Care Management entails: Telephone Reassurance SASSFA staff and volunteers are available to provide daily telephone calls (Monday through Friday) to monitor the well-being of homebound seniors who live alone. In-home ASSESSMENT of physical, psychological, and health needs by a professional social worker DEVELOPMENT of an individual care plan specific to their needs COORDINATION and arrangement of services Monitoring and FOLLOW-UP
